From your GP or medical specialist, addressed to specialist mental-health care (S-GGZ), with the primary question clearly stated.
A valid passport, Dutch ID card or residence permit. We verify at intake — upload isn't needed beforehand.
Your citizen service number (BSN) and the details of your basic health-insurance policy for the current year.
Plan ± 15 minutes for the form, in a spot where you can think calmly. You can save and continue later.

Your GP can advise on a more suitable service.
The usual route. Your GP writes a referral to specialist mental-health care and gives it to you — on paper or via ZorgDomein.
Most basic health insurers require a GP referral for reimbursement.
Already under the care of a medical specialist (e.g. psychiatrist, neurologist) who refers on? Then a specialist referral is sufficient.
No referral at all? Please contact your GP first.
The form is hosted by our parent institution, De GGZ Coöperatie, on an end-to-end encrypted portal that meets NEN 7510. Nothing is shared with third parties.

A short email confirming receipt. If something is missing we call you back the same evening.
A practitioner reviews your form and referral. You'll hear whether we accept the application and when intake is planned.
The first video consultation. After that, the current treatment waiting time after intake applies as listed on the waiting-times page.
Yes. A GP or medical specialist must refer you to specialist mental-health care. Without a referral your treatment isn't reimbursed by your basic insurance.

Only if you are a formal legal representative. In all other cases the application must come from the person themselves — also by email or phone.
ADHD diagnostics and treatment may be insured specialist mental-health care when there is a valid referral, suitable care need and insured DSM classification or indication. See Fees & reimbursement for details.
