GP referral letter for ADHD — how does it work?
To receive treatment at AvondFocus you need a referral letter from your GP. This page explains the process step by step.
Step by step
Make an appointment with your GP
Call or book an appointment with your GP online. Mention that you would like a referral for specialised mental health care due to a suspected ADHD diagnosis. You do not always need a lengthy consultation — many GPs are familiar with ADHD and will refer you promptly.
Your GP refers via ZorgDomein
Your GP can look up AvondFocus directly in ZorgDomein and send a digital referral. If your GP does not use ZorgDomein, a referral letter on paper or via secure e-mail is also possible. Ask your GP to include our AGB code in the referral.
We receive the referral and contact you
After receiving your referral and your registration via our registration form, we will contact you within five working days. We will schedule an intake appointment on a Wednesday evening via secure video connection.
Contents of the referral letter
Your GP will know the requirements. For reference, the necessary details are listed below.
Personal details
Full name, date of birth, address, telephone number and e-mail address of the patient. The BSN (citizen service number) and insurance details are also needed for administration.
Complaints and reason for referral
A brief description of the complaints and the reason for referral to specialised mental health care. For example: "concentration difficulties, impulsivity, suspected ADHD, requesting diagnostics and treatment."
AGB code of AvondFocus
Your GP must include the AGB code of AvondFocus in the referral. Without a valid AGB code, the health insurer cannot link the referral. Ask your GP to look up the AGB code in ZorgDomein or contact us directly.

Frequently asked questions
Can I refer myself to AvondFocus?
No. For insured specialist mental-health care you need a referral letter from an authorised referrer: your GP, occupational physician, medical specialist or youth physician. Without a valid referral, your health insurer may not cover the treatment.
Does the referral have to go through ZorgDomein?
Preferably yes — ZorgDomein is the fastest route. Your GP can refer directly to AvondFocus via ZorgDomein. If your GP does not use ZorgDomein, a referral letter on paper or via secure e-mail is also possible. Please contact us for the correct procedure.
How long does it take before I hear back?
After receiving your referral and your registration, we aim to contact you within five working days. Please check our current waiting times page for an indication of the timeline until your intake appointment.
