AvondFocus is listed in ZorgDomein. On this page you will find the inclusion criteria, referral process and contact details for referrers.
Indications
Check the patient against the criteria below before referring. If in doubt, you are welcome to consult us by e-mail.
Inclusion criteria
- Age 18 or older
- Suspected ADHD based on history or screening questionnaire (e.g. DIVA, ASRS)
- Complaints have a significant impact on daily functioning (work, relationships, self-management)
- Patient is able to participate in online treatment via video connection
- Patient is motivated for diagnostics and potential treatment
Exclusion criteria
- Active suicidality or acute safety risks
- Severe comorbid psychiatric conditions as the primary care need (e.g. psychosis, severe personality disorder)
- Severe substance misuse as the primary concern
- Age under 18
- Insufficient command of Dutch for diagnostic interviews

Find AvondFocus in ZorgDomein
Open ZorgDomein, select "Specialised mental health care" and search for "AvondFocus". Select the practice.

Complete and send the referral
Enter the reason for referral and relevant history. ZorgDomein guides you through the required fields. Advise the patient to also register via avondfocus.nl.

We contact the patient
The patient receives confirmation and we schedule the intake. You will receive feedback after intake and at discharge.
Frequently asked questions
Can I also refer without ZorgDomein?
Yes. If your practice does not use ZorgDomein, you may provide a referral letter to the patient or send it via secure e-mail (compliant with NEN 7510). In that case, please contact us at info@avondfocus.nl so we can send you the correct procedure.
Which patients are suitable for AvondFocus?
AvondFocus treats adults aged 18 and over with a suspected or confirmed ADHD diagnosis. Treatment is delivered online on Wednesday evenings. We are not suitable for patients with active suicidality, severe comorbid psychiatric conditions requiring acute treatment, or severe substance misuse as the primary concern.
Will we receive feedback after the intake or treatment?
Yes. After completion of the intake and at the end of the treatment episode, we send a structured feedback report to the referring GP, in accordance with NHG guidelines and privacy regulations. The patient consents to this at registration.
